On August 21, 2026, the Minister of Finance issued Circular 121/2026/TT-BTC amending and supplementing a number of articles of Circular 68/2025/TT-BTC, which issued forms used for enterprise registration and business registration. Accordingly, the Appendix issued with Circular 121/2026/TT-BTC replaces certain forms used in enterprise registration, specifically as follows:

• Replacing Form No. 1 of Appendix I issued with Circular 68/2025/TT-BTC with Form No. 1 (Application for enterprise registration – Private enterprise) in the Appendix issued with Circular 121/2026/TT-BTC;
• Replacing Form No. 2 of Appendix I issued with Circular 68/2025/TT-BTC with Form No. 2 (Application for enterprise registration – Single-member limited liability company) in the Appendix issued with Circular 121/2026/TT-BTC;
• Replacing Form No. 3 of Appendix I issued with Circular 68/2025/TT-BTC with Form No. 3 (Application for enterprise registration – Limited liability company with two or more members) in the Appendix issued with Circular 121/2026/TT-BTC;
• Replace Form No. 4 in Appendix I issued with Circular 68/2025/TT-BTC with Form No. 4 (Application for enterprise registration – Joint-stock company) in the Appendix issued with Circular 121/2026/TT-BTC;
• Replace Form No. 5 in Appendix I issued with Circular 68/2025/TT-BTC with Form No. 5 (Application for enterprise registration – Partnership) in the Appendix issued with Circular 121/2026/TT-BTC;
• Replace Form No. 10 in Appendix I issued with Circular 68/2025/TT-BTC with Form No. 10 (List of beneficial owners of the enterprise) in the Appendix issued with Circular 121/2026/TT-BTC;
• Replace Form No. 12 in Appendix I issued with Circular 68/2025/TT-BTC with Form No. 12 (Application for registration of changes to the Enterprise Registration Certificate/Notification of changes to enterprise registration information – Used for: registering changes to the Enterprise Registration Certificate/notification of changes to enterprise registration information; supplementing or updating enterprise registration information; requesting correction of enterprise registration information) in the Appendix issued with Circular 121/2026/TT-BTC;
• Replace Form No. 27 in Appendix I issued with Circular 68/2025/TT-BTC with Form No. 27 (Notice of suspension of business/resumption of business prior to the notified deadline by an enterprise/branch/business location, or notice of suspension of operations/resumption of operations prior to the notified deadline by a representative office) in the Appendix issued with Circular 121/2026/TT-BTC;
• Replace Form No. 30 in Appendix I issued with Circular 68/2025/TT-BTC with Form No. 30 (Notice of enterprise dissolution) in the Appendix issued with Circular 121/2026/TT-BTC;
• Abolish Form No. 11 in Appendix I issued with Circular 68/2025/TT-BTC.
